چکیده

Introduction: Patients with type 2 diabetes mellitus (DM) are at increased risk of dying from cardiovascular diseases. Elevated cardiovascular risk in this population persists even after other conventional cardiovascular risk factors are eliminated or reduced, suggesting that there are other, incompletely understood, mechanisms which are increasing cardiovascular risk in such patients. Ventricular instability, such as that observed in QT abnormalities, may be an important additional mechanism. QT prolongation has been shown to predict cardiac death in type 2 DM. The aims of our study were to evaluate the prevalence of QTc prolongation in patients with type 2 DM and to assess the relationship between insulin resistance, evaluated by HOMA-IR and QTc. We also investigated the relationship between QTc and the duration of DM, HbA1c, plasma lipids, systolic blood pressure (SBP), diastolic blood pressure (DBP) and body mass index (BMI). Materials and method: We enrolled 104 patients with type 2 DM. For each subject, 100 consecutive heart beats were recorded on the resting ECG tracing. QTc was calculated according to Bazett’s formula. Results and discussion: In patients with type 2 DM, the prevalence of a prolonged QTc interval was 34.6%. We found a significant increase, with 64 ms (p= 0.004) in QTc interval for the insulin resistant cohort (mean QTc= 419 ms; n= 92) compared to the sample with normal HOMA IR (mean QTc= 355; n=12). European Scientific Journal April 2013 edition vol.9, No.12 ISSN: 1857 – 7881 (Print) e ISSN 18577431 71 Conclusion: Insulin resistance, estimated by HOMA-IR, was strongly correlated with prolonged QTc. Prolonged QTc identifies type 2 DM patients with an elevated risk of cardiovascular events.

Prevalence and Risk Factors of Prolonged QTc Interval among Chinese Patients with Type 2 Diabetes

OBJECTIVES The aim of this study was to evaluate the prevalence and the risk factors of prolonged QTc interval among Chinese patients with type 2 diabetes. METHODS The retrospective study included 3156 outpatients from the Diabetes Centre, the 306th Hospital of PLA, during the period from September 2003 to June 2010. QT interval was measured manually in the 12-lead conventional electrocardiog...

The relationship between insulin sensitivity and heart rate-corrected QT interval in patients with type 2 diabetes

BACKGROUND Reduced insulin sensitivity not only contributes to the pathogenesis of type 2 diabetes but is also linked to multiple metabolic risk factors and cardiovascular diseases (CVD). A prolonged heart rate-corrected QT interval (QTc interval) is related to ventricular arrhythmias and CVD mortality and exhibits a high prevalence among type 2 diabetes patients. The aim of the study was to in...

Baseline-Corrected QT (QTc) Interval Is Associated with Prolongation of QTc during Severe Hypoglycemia in Patients with Type 2 Diabetes Mellitus

BACKGROUND We investigated an association between baseline heart rate-corrected QT (QTc) interval before severe hypoglycemia (SH) and prolongation of QTc interval during SH in patients with type 2 diabetes mellitus (T2DM). METHODS Between January 2004 and June 2014, 208 patients with T2DM, who visited the emergency department because of SH and underwent standard 12-lead electrocardiography wi...

The effect of 25-hydroxyvitamin D levels on QT interval duration and dispersion in type 2 diabetic patients

AIM To assess the relationship between corrected QT (QTc) interval and vitamin 25-hydroxyvitamin D levels (25-OHD) deficiency in type 2 diabetic patients. METHODS The study included 253 patients with type 2 diabetes and 170 age-matched controls treated between October and December 2013. QTc duration and QTc dispersion were measured on ECG recordings and 25-OHD, calcium, phosphorus, and blood ...
